• Return Mailing Address
• Conversion instructions (i.e. OtoPulse to OtoScreen III)

Either click the "Buy Type Conversion" button on the left and pay online or include a check in the box with your unit.

If paying by check:

Include a check for $59 ($49 plus $10 shipping and handling) for just a type conversion.

Include a check for $119 ($109 plus $10 shipping and handling) for a type conversion including a re-calibration service.

The re-calibrations service is recommended when changing unit types.

Units are not returned until payment is received.
